The DCHCP comprehensive health insurance program is a benefit offered to Ohio members of the American Dental Association. The member dentist may also offer the plan to their dependent family members and their dental office staff (must have proof of employment = 25 hours per week minimum).
To begin the process or to receive a quote:
1) Complete all enrollment forms & questionnaire using a ball point pen.
